Norfolk Family Health Team Move To Simcoe – Q & A

The Norfolk Family Health Team is Steps Away in Paris
Brantford/Brant, ON. – The Chronic Non-Cancer Pain Management Program (CNCPMP) with The Norfolk Family Health Team, is currently providing medical services at 363 Colborne Street, in Brantford at the Grand River Community Health Centre (GRCHC), and is moving to the Cowan Community Health Hub. This short, local move, will realize the major benefit of having additional medical services that are easily accessible, at this new location, 25 Curtis Ave N, in Paris.

The new home of the CNCPM Team is completely accessible, has adjacent parking, is cheerfully bright with natural light, and is extremely welcoming. We are very fortunate Grand River Community Health Centre (GRCHC) offered us this location, as they continue to expand primary care delivery in the downtown core of Brantford. Most importantly the CP team, we will remain open, with the very same weekly schedule that our patients have come to expect, just at our new location.

The new location located in the Cowan Community Health Hub, at 25 Curtis Ave N, Paris, is set to open in May 27th, 2024. The Brantford location of the Pain Clinic will be closed on May 23rd and May 24th. Our contact information will all stay the same. We are sorry for the temporary inconvenience while we move locations.
