Delhi Location: With deep roots in our vibrant community, the Delhi Community Health Centre is home to the Norfolk Family Health Team’s flagship location. Visit us at 105 Main Street.
Port Rowan Location: Located in the picturesque waterfront town of Port Rowan. Visit us at the Port Rowan Medical Centre, 1009 Bay Street.
The delivery of our Programs & Services encompasses all of Norfolk County including: Delhi, Port Rowan, Simcoe, Waterford, Port Dover, and all other surrounding Norfolk communities.
