The Norfolk Family Health Team (NFHT) is a community based not-for-profit organization dedicated to providing quality, person-centered health care for individuals and families residing in Norfolk County. 

NFHT provides access to family physicians, nurse practitioners, midwives, nurses, social workers, dietitian, pharmacist and other health professionals. NFHT provides specialized services for rostered patients, including the Diabetes Management Program, Geriatric Program, Registered Dietitian, Mental Health Counseling, Cancer Screening, Well baby and Childhood Immunization, Spirometry Clinic, and Pharmacy Medication Reconciliation. Community Programs include the Chronic Non-Cancer Pain Management Program, Norfolk Prenatal and Newborn Health Program, Sexual Health Program, Seasonal Agriculture Workers, Foot Care and Smoking Cessation. 

A secondary site can be found in Port Rowan and the Grand River Community Health Centre in Brantford. 

The Chronic Non-Cancer Pain Management Program serves Haldimand, Norfolk and Brant, with sites in Haldimand, Norfolk and Brant which provides interprofessional primary care interventions to support individuals with reducing their prescription opioid use and chronic pain. 

The interprofessional primary care team will be comprised of Nurse Practitioners, Nurses, Social Workers, kinesiologist Occupational Therapist and medical office assistant, who have expertise in pain and chronic disease management. 

An immediate need exists for one 30 hour/week Occupational Therapist. 

General Overview: 

The Occupational Therapist (OT) with the Norfolk Family Health Team works alongside the interprofessional primary care team, providing assessment and treatment/program planning. The OT provides education and strategies, for individuals living with chronic pain and chronic disease, to enhance their ability to safely and independently complete their daily activities (Activities of Daily Living). 

Responsibilities: 

The Occupational Therapist: 

• is responsible for providing assessment of presenting impairment, in a community and/or home setting, using standardized assessment tools. 
• is responsible for designing a treatment/program plan using evidence-based information, relevant Best Practice guidelines and clinical expertise. 
• actively engages the patient, family and/or caregiver to effectively address the patient’s identified goals. 
• is responsible for working collaboratively with the patient and interprofessional primary care team to develop a plan, including short and long-term patient goals, and to provide support and interventions in alignment with the same. 
• engages in ongoing evaluation and documentation of the effectiveness of the intervention and the patient’s response and progress towards goal attainment. 
• is responsible for the delivery of services in a 1-1 setting and through group facilitation. 
• is responsible for facilitating system navigation and the provision of information and referral to appropriate services. 
• reports to the Executive Director and works closely with other members of the interprofessional primary care team.

Requirements: 

• Master of Science in Occupational Therapy 
• Registered member of the College of Occupational Therapists of Ontario in good standing 
• Member of the Ontario Society of Occupational Therapists 
• Current certification and maintenance of CPR for Health Care Providers 
• Experience working with individuals living with chronic pain 
• Experience in chronic disease management and knowledge of patient self-management strategies 
• Strong clinical, organizational and interpersonal skills 
• Committed to Best Practice in patient-centered care 
• Exceptional documentation skills and attention to detail 
• Exceptional communication skills 
• A positive and professional attitude 
• Excellent time management skills with an ability to multi-task and effectively set priorities 
• Demonstrated ability to work independently as well as a part of a diverse and interprofessional primary care team 
• Flexibility and adaptability 
• Experience in new program development and program evaluation an asset 
• Experience in and/or familiarity with rural medicine an asset 
• Familiarity and comfort working within an EMR system an asset 
• Multilingualism (e.g., French, Polish, Hungarian, Low German) an asset
