Dr. Margaret Bak will be retiring from practice effective September 1, 2026. It has been Dr. Bak’s privilege to provide care to you over the years, and we appreciate the trust you have placed in her. This decision was not made lightly, and the Norfolk Family Health Team is committed to supporting patients during this transition.
Ongoing Care
At this time, there will not be a replacement physician. We are actively working to recruit another physician to join our team. In the meantime, patients may access care through:

If the Norfolk Family Health Team secures a replacement physician or nurse practitioner, future patient attachment will occur through the Health Care Connect waitlist. The Norfolk Family Health Team no longer maintains an internal waitlist, so patients must be registered with Health Care Connect to be considered.
Care Until Retirement
Dr. Bak will continue to provide care until August 28, 2026. Patients are encouraged to book any necessary appointments, renew prescriptions, and discuss ongoing healthcare needs before that date.
Medical Records
Your medical records will remain confidential and secure. Once you have a new healthcare provider, records can be transferred with your signed consent. For medical record requests or transfers, please contact the Norfolk Family Health Team:
