Psychiatry Clinic — Dr. Clements, MD, Psychiatrist

Clinic Overview:

• Dr. Clements, Psychiatrist (MD, specialized in Psychiatry & Addictions), holds clinics once per month
• Provides care for FHT patients concerned with:
  • Mood disorders
  • Anxiety
  • Cognitive concerns in older adults
  • Addiction-related mental health issues

Eligibility:

• FHT patients only
• Must be referred by a physician or practitioner
• 🚫 No ADHD referrals accepted

Clinical Workflow

Specialized Psychiatry Nurse:

• Conducts pre-assessments and intake forms
• Reviews labs and relevant medical history
• Contacts patients and families for initial information
• Manages follow-up appointments
• Provides ongoing patient and family support
• Dr. Clements sees the patient for assessment, diagnosis, and treatment planning

Collaborative Partners
