Geriatrics Clinic — Dr. Braun, MD, Geriatrician

Clinic Overview:

• Dr. Braun, Geriatrician (MD, Fellowship in Geriatrics), holds clinics twice per month from April to November
• Provides care for FHT patients concerned with:
  • Parkinson’s disease
  • Memory loss / cognitive decline
  • Frailty
  • Other geriatric conditions (falls, polypharmacy, functional decline)

Eligibility:

• FHT patients only
• Must be referred by a physician or nurse practitioner

Clinical Workflow

Specialized Geriatric Nurse:

• Conducts pre-testing (MOCA, cognitive screening)
• Reviews labs & diagnostic imaging
• Contacts patients and families for initial information
• Manages follow-up appointments
• Provides ongoing patient and family support

Collaborative Partners

• Close collaboration with the Alzheimer Society, present on in-clinic days
• Team-based approach ensures comprehensive assessment and continuity of care
