Norfolk Family Health Team, located in Norfolk County, Ontario, provides comprehensive primary care and allied health services through a collaborative, interdisciplinary team. We are currently seeking two Primary Care Physicians to join our dynamic and growing team.
The Norfolk Family Health Team brings together Physicians, Nurse Practitioners, Registered Nurses, Pharmacists, Social Workers, Midwives, Kinesiologists, and Medical Office Assistants. Physicians maintain a patient roster while working collaboratively with an experienced interdisciplinary team to deliver high-quality, coordinated care. No investment or overhead is required — we provide the facility, administrative support, and resources needed to establish and grow your practice quickly.

We are seeking a full-time physician (1 BSM FTE) to join our team immediately. This position is ideal for a physician looking to establish or expand a long-term practice within a supportive, team-based model.
Requirements
We welcome both new graduates and experienced physicians interested in transitioning to a collaborative care model that supports a healthy work-life balance. Physicians joining the Norfolk Family Health Team will:
- Provide comprehensive primary care services to a rostered patient population
- Work collaboratively within an interdisciplinary team
- Utilize EMR and digital tools to support coordinated care
- Participate in quality improvement and team-based care initiatives
- Commit to building and growing a patient roster
- Flexible scheduling options
- Ability to work at one site or multiple locations
Hospital Opportunities
Additional opportunities are available at our local hospital, located just a two-minute walk from our clinic, providing flexibility for physicians interested in hospital-based work.
Benefits for New Graduates & Physicians New to Ontario
New graduates and physicians new to Ontario are eligible for income stabilization funding for up to one year, in exchange for committing to develop a practice and build a patient roster. Following this period, physicians transition to the Blended Salary Model.
