For Norfolk FHT patients individuals are provided counseling and Nicotine Replacement.

For unattached patients our program helps you with your quit path journey by connecting individuals with great community programs such as the OMSC Community Program Community Program | Ottawa Model for Smoking Cessation (ottawaheart.ca) which provides 6 weeks of Nicotine Replacement, STOP on the Net Quit Smoking with STOP on the Net | Nicotine Dependence Services (nicotinedependenceclinic.com), Telehealth Ontario Smoking Cessation Services Health811 – Health811 (ontario.ca), Smokers’ Helpline Home (smokershelpline.ca) and Quash Home (quashapp.com).

The Norfolk Family Health Team is Funded by the Ministry of Health to provide sustainable rural healthcare in Norfolk County.
