Chronic Non-Cancer Pain Management Program
The Chronic Non-Cancer Pain Management Program (CNCPMP) at Norfolk Family Health Team offers compassionate, team-based support for individuals living with long-term pain. This program is especially helpful for those who have come to understand that their pain is ongoing and may not be cured—but can be managed with the right tools, guidance, and care.
What the Program Strives to Achieve
The Chronic Non-Cancer Pain Management Program is designed to help individuals move forward with strength and support. Our key goals include:
- Helping reduce the intensity and impact of chronic pain
- Supporting a return to work or other meaningful daily activities
- Addressing emotional and social challenges linked to pain—such as anxiety, low mood, sleep issues, and strained relationships
- Supporting the safe and gradual tapering of narcotic pain medications
- Promoting greater independence and reducing frequent reliance on the healthcare system
Through education, movement, and compassionate care, we empower patients to take back control and live well—despite chronic pain.
The CNCPMP is described as an interdisciplinary program, meaning it brings together a team of healthcare professionals, Nurse Practitioners, Nurses, Kinesiologists, Social Workers, and a Dietitian to support individuals living with long-term pain.
The CNCPMP empowers patients to take an active role in managing their pain. While medical treatments and procedures have their place, the program focuses on building the skills and confidence needed to live a full life despite chronic pain. We don’t promise a cure; we offer a way forward.
